Out of Hours Care Coordinator – South Tyneside

If so then join our team as an Out of Hours Care Coordinator and make a meaningful difference in the lives of individuals in your community by supporting people to live safely and independently in their own homes, bringing joy and improving their quality of life.
Shift Pattern As Below:
Week one working Monday, Tuesday & Wednesday working 0600am to 0900am then 1700pm to 2300pm (split shift)
Week two working Saturday & Sunday working 0600am to 1400pm
Optional extra hours are available (as a Home Care Worker)
Roles and Responsibilities
Coordinate and manage care services
Respond to emergency calls and provide immediate assistance as needed
Schedule and allocate care staff to meet client needs
Maintain accurate records of all care activities and communications
Liaise with clients, families, and healthcare professionals to ensure seamless care delivery
Provide support and guidance to care staff during out-of-hours shifts
Home care calls will be required as part of the role
